Lose your COVID-19 vaccination card and need a replacement?
We only process replacement cards for individuals who received their vaccine from the City of Hamilton Health Department or those who live within the city limits of Hamilton. If you received your vaccine from another facility and need a replacement card, contact the facility or state you received your vaccine in.
